Abstract
Purpose: To evaluate upper sacral m orphology and anatom y of safe zone related to iliosacral screw fixation in Korean.

Materials and Methods: 100 patients perform ed pelvis 3D C T scan were evaluated. W e used 16 channel C T and analyzed reconstructed im age (shaded-surface display, transparent im age and reform at im age).

Results: The angle betw een superior aspect of S1 body and iliac cortical density is 27.3o, betw een anterior cortical line of S1,2 body and horizontal plane 24.6o, and betw een superior aspect of S1 body and horizontal plane is 39.7o. The axis of S1, S2 pedicle is 32.5o and 15.6o tow ard anterom edial. The area of S1 pedicle according to sagittal plane and sagittal-oblique axis is 310.7 m m 2 and 384.8 m m 2. Also, S2 pedicle area is increased 163.1 m m 2 to 188.4 m m 2. The average depth of ala indentation is 5.1 m m and the m axim al value is 9.5 m m . Distinct upper sacral dysplasia is 22% , transitional form is 32% .

Conclusion: W e m easured Korean upper sacrum w ith 3D-C T, found out dysplasia com e up to 54% . Considering the frequency of dysplasia, the investigation of anatom y and technique is essential to sacroiliac screw insertion.
